Zusammenfassung
Der Begriff (Gesundheits-)Erstschaden ist ein juristischer, er gehört jedoch zum sogenannten medicolegalen Kontext, der auch dem ärztlichen/psychologischen Gutachter geläufig sein muss. Es wurde versucht, dieses Grenzgebiet medizinischer/psychologischer und juristischer Kompetenz in der Begutachtung aus der Sicht des medizinischen Sachverständigen zu erläutern und auf die Vorgaben der internationalen Diagnosesysteme ICD-10, DSM-IV-TR und DSM-5 zu beziehen. Eine Bezugnahme auf die ICD-11 erfolgte an dieser Stelle lediglich am Rande, da diese erst zum 01.01.2022 zu erstem medizinischem Gebrauch zugelassen werden wird. Der gutachtliche Umgang mit dem Störungsbild komplexe posttraumatische Belastungsstörung wurde thematisiert und anhand eines Fallbeispiels erläutert.

Posttraumatic stress disorder and direct damage from a medical perspective
Abstract
The judicial term “Gesundheits-)Erstschaden” means “direct damage (to health)” and is used in a medicolegal context with which the medical/psychological expert must be familiar. This field encompasses medical/psychological and legal competence in assessment and attempts have been made to explain it from the perspective of the medical expert with reference to the requirements of the international diagnostic systems ICD-10, DSM-IV-TR and DSM-5. The paper makes only passing reference to ICD-11 as this will not come into effect until 01.01.2022. It picks out as a central theme the expert handling of the clinical picture of complex posttraumatic stress disorder and uses a case study by way of illustration.
